Adoption Health Records Review(AHRR) and myObMD Potential Adoptive Parents Agreement

I/We are hiring Adoption Health Records Review to perform a pre-adoption review of prenatal records only.

I/We Understand that:

• No assessment, whether in person or virtual,  of the birth mother or fetus will take place.
• The review is strictly of the prenatal records only.
• Adopt Health Records Review is willing to review reports of routine ultrasounds performed as part of the prenatal records; however, a review of the images will not be performed.
• I/We (potential adoptive parent(s) are responsible for providing Adoption Health Record Review with the prenatal records we wish to be reviewed.
• Adopt Health Records Review will have NO contact with the owner (birth mother) of the prenatal records.
• I/We (adoptive parents) acknowledge we have legally and ethically acquired the prenatal records we wish to be reviewed.
• I/We (adoptive parents) acknowledge that the accuracy of the Adoption Health Records Review Report you will receive will be based on the accuracy of the prenatal records you have provided. Adoption Health Records Review is not responsible for inaccurate or omitted information in the prenatal records.

Services Available at Additional Cost: 

• Review of high-risk Ob (MFM) prenatal visit – $100/visit
• Additional Video Meetings – $150/30 min
• Phone Consultation – $75/30 min
